To ensure a safe and timely experience we are encouraging individuals and families to complete their vaccination forms ahead of time and bring each individual's license (if applicable) and insurance cards with them to the clinic.

The required forms can be found below. Please print and complete the consent form and bring all completed forms with you to the flu clinic.

 Vaccines will be given on a first come, first served basis. All clinics are subject to vaccine availability and supply.

• Please bring an insurance card and ID.
  

• UNITED HEALTHCARE DOES NOT RECOGNIZE STRATFORD HEALTH DEPARTMENT AS A PROVIDER.
